SCHEDULING YOUR FIRST VISIT
If you have not already scheduled your appointment with us, please call the office at (208) 255-1951 to do so. If you prefer to email us and have the clinic contact you regarding an appointment or question, please send your message to info@greenmtmed.com.
DOWNLOADING FORMS FOR YOUR FIRST VISIT
Each new patient needs to open and read our Welcome Letter, a letter introducing you to our office policies and procedures. To open this letter, click on the Welcome Letter link below. Feel free to download the letter and save a copy for your records.
We also ask that you download, print and complete a health history form to bring with you to your first visit. Completing the forms at home allows you to take your time, and gives you convenient access to your health records as well as supplement and/or medication information. Please be sure to allow between 15 and 30 minutes to complete the form.
Select the appropriate form below according to the type of appointment scheduled and the patient’s age:
Health History Form – 13 years to adult
Pediatric History Form – birth to 5 years
Pediatric History Form- 6 to 12 years
How Do I Check My Insurance Benefits?
If you are unable to download, read or print any of these files, we would be happy to email, fax or mail them to you.
YOUR APPOINTMENT
Remember to bring your completed forms with you to your first visit. We also ask that you bring any supplements or medications you are taking regularly, and copies of any relevant laboratory results or doctor’s notes. If you have any questions about your upcoming visit, don’t hesitate to call our clinic at (208) 255-1951 or send an email message to info@greenmtmed.com.
If for any reason you need to cancel or reschedule your visit, please give us at least 24 hours notice. Out of respect for our time and to other patients waiting for visits, a missed appointment fee will be charged without proper notice. Thank you for your courtesy, and we look forward to seeing you in our clinic soon.
